No. While many of our clients are older adults and their families, ClearPath may also assist individuals with disabilities, caregivers, veterans and their families, and others navigating Medicaid long-term services and supports or other benefit programs.

Long-Term Care Medicaid helps eligible individuals access long-term services and supports. Depending on the person's needs and eligibility, services may be provided in a nursing facility or through qualifying Home and Community-Based Services.
Eligibility involves more than income alone and may include medical/functional and financial requirements.

Depending on the service selected, ClearPath can assist with preparing and submitting a Long-Term Care Medicaid application, organizing supporting documentation, responding to requests for information, communicating with the appropriate agencies when authorized, and helping you understand the determination process.

Possibly. Medicaid eligibility depends on the type of Medicaid being requested and the individual's specific circumstances. Being over an income limit does not automatically mean someone cannot qualify for Long-Term Care Medicaid.

The Uniform Assessment Instrument (UAI) is used in Virginia as part of evaluating an individual's needs for certain long-term services and supports.
The screening process evaluates functional and other criteria to determine whether an individual meets the applicable level-of-care requirements.

No. An approved UAI and Medicaid financial eligibility are separate parts of the process.
A person may meet the functional requirements for long-term care services but still need to meet Medicaid's applicable financial and other eligibility requirements.

Consumer Directed Services allow qualifying individuals to have greater choice and control over who provides their approved care services. Rather than receiving care exclusively through a traditional home care agency, an eligible individual may be able to select and hire a person of their choosing to provide certain authorized services.
This can be especially helpful when a family member, friend, or other trusted individual is already providing unpaid care. If that person meets the applicable program requirements and is approved to serve as the paid attendant, Medicaid may compensate them for providing authorized services they may already be helping with for free.
Consumer Directed Services can give individuals and families greater flexibility, choice, and control over how care is provided while allowing eligible caregivers to be paid for approved services.
